How to Participate in the Walmart Survey?
- Visit any participating Walmart store and make a purchase to receive a receipt.
- Now go to the Walmart survey website: www.survey.walmart.com.
- Look for the unique survey code printed at the bottom of your receipt.
- Select your preferred language (English or Spanish).
- Enter your survey code from your receipt.
- Respond to the survey questions based on your recent shopping experience.
- Once completed, click on the "Submit" button.
- Now, complete the entry form which include your name, street address, city, state, zip, phone number and email address and click “Continue” button.
- Then, complete the check boxes and click “Submit” to receive one (1) entry into the Sweepstakes.
